In-Home Healthcare LVN - Part Time or Full Time - Days/Nights

About the role

We are seeking compassionate Licensed Vocational Nurses (LVNs) to provide skilled nursing care in home settings, primarily for pediatric patients with feeding tubes and medication needs. Responsibilities

• Delivering and documenting patient care according to physician orders and care plans.
• Monitoring and assessing patient conditions, reporting changes promptly.
• Administering medications, managing feeding tubes, and implementing nursing actions to achieve positive outcomes.
• Educating patients and families on disease management, self-care, and prevention.
• Maintaining accurate documentation and adhering to safety and infection control standards.
• Participating in ongoing education and professional development.
• Providing high-quality, compassionate care in diverse home environments.
• Following safety protocols, infection control, and compliance with regulations.
• Documenting activities accurately and timely.
• Collaborating with the care team to meet patient needs.

Qualifications

• Active, unencumbered LPN/LVN license in the practicing state.
• Current CPR certification and TB test within the last 12 months.
• Preferred: Six months of recent clinical experience; home health experience is a plus.
• Reliable transportation and physical ability to lift up to 50 pounds, reposition patients, and perform necessary mobility tasks.
