Part Time In-Home Healthcare LVN

About the role

We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in a home setting, primarily on weekends.

Responsibilities

• Delivering and documenting patient care in accordance with physician orders and care plans.
• Monitoring patient conditions, assessing needs, and reporting changes promptly.
• Educating patients and families on disease management, self-care, and prevention.
• Ensuring infection control and safety standards are maintained.
• Participating in ongoing education and maintaining compliance with policies and regulations.

Qualifications & Skills

• Active, unencumbered nursing license in the state of practice.
• Current CPR certification; additional state-specific experience requirements may apply.
• Reliable transportation and physical ability to lift and reposition patients.
• Strong attention to detail, organizational, and communication skills.
• Ability to work independently in varied environments with exposure to infectious materials.
