In-Home Healthcare LVN - Full Time/Part Time

About the role

We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in a home setting, primarily for pediatric patients with high acuity needs.

Responsibilities

• Deliver and document patient care according to physician plans
• Assess patient conditions and report changes promptly
• Update care plans and implement nursing actions
• Educate families on care procedures and protocols
• Ensure infection control standards are met
• Adhere to safety protocols in environments with potential exposure to infectious materials

Requirements

• Active, unencumbered LPN/LVN license in the practice state
• Current CPR certification
• TB test within the last 12 months
• Reliable transportation
• Physical ability to lift patients, move equipment, and respond to emergencies

Qualifications

Preferred candidates have at least six months of recent clinical experience, with additional skills in assessments, documentation, and communication.
