About the role

We are seeking a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care in a home setting, primarily for pediatric patients. This role involves delivering and documenting care according to physician orders, assessing patient conditions, updating care plans, and educating families on disease management and self-care techniques. The LVN will coordinate with supervising nurses, recognize changes in patient needs, and implement appropriate interventions to ensure safety and health outcomes.

Responsibilities

• Providing quality patient care and documenting activities accurately
• Monitoring and reporting patient condition changes
• Participating in care plan updates and infection control practices
• Educating patients and families on care techniques and disease prevention
• Adapting to diverse environments and responding effectively to emergencies

Qualifications

• Active, unencumbered LVN license in the practicing state
• Current CPR certification
• TB test within the last 12 months
• Reliable transportation
• Preferably six months of recent clinical experience; pediatric experience is a plus
• Compliance with health regulations, including vaccinations and testing, as required by jurisdiction

• Physical ability to lift up to 50 pounds and perform patient repositioning
• Proficiency in documentation and compliance with policies
