In Home Healthcare LVN - Full-Time

About the role

We are seeking a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care in a home setting, primarily for pediatric patients with complex needs such as feeding tubes and medication administration.

Responsibilities

• Deliver and document patient care according to physician orders and care plans
• Monitor and assess patient conditions, reporting changes promptly
• Implement nursing interventions to achieve desired outcomes
• Educate patients and families on disease management and self-care
• Adhere to confidentiality, infection control, and safety protocols
• Maintain accurate documentation of activities and assessments
• Provide high-quality, compassionate nursing care in patients’ homes
• Observe, assess, and respond to patient needs and changes in condition
• Document all care activities accurately and timely
• Educate families and patients on self-care and safety measures
• Follow all regulatory, safety, and infection control standards
• Work in diverse environments, travel as needed, and respond effectively to emergencies

Requirements

• Active, unencumbered state LVN/LPN license
• Current CPR certification
• TB skin test within the last 12 months
• Reliable transportation
• Physical ability to lift up to 50 pounds and perform patient repositioning
• Strong communication, organization, problem-solving, and time management skills

Qualifications

• Preferred: six months or more of clinical nursing experience
• Preferred: home health background
