Part Time In-Home Healthcare LVN

Responsibilities

• Delivering and documenting patient care based on physician orders and care plans
• Monitoring and assessing patient conditions, reporting changes promptly
• Implementing nursing actions to achieve desired outcomes and prevent risks
• Educating patients and families on disease management and self-care techniques
• Adhering to infection control standards and maintaining compliance with policies, regulations, and the Nurse Practice Act
• Participating in ongoing education and skill development

Requirements

• Active, unrestricted LPN/LVN license in the practice state
• Current CPR certification and TB test within the past 12 months
• At least 6 months of clinical nursing experience, preferably in home health (preferred)
• Effective communication, organization, problem-solving, and attention to detail skills
• Reliable transportation and physical ability to lift up to 50 pounds and reposition patients
• Ability to work in diverse environments, with exposure to bodily fluids and potential allergens
• Proficiency in English, with good visual and hearing acuity, and capacity to respond to emergency situations
