In Home Healthcare LVN - Full Time Days or Nights

We are seeking a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care in a home setting for pediatric patients with complex needs, including feeding tube management and medication administration. Key Responsibilities Include Delivering and documenting patient care according to physician orders and care plansAssessing patient condition, recognizing changes, and reporting to supervisors or physiciansEducating patients and families on disease management, self-care, and preventionMaintaining confidentiality, professional boundaries, and compliance with regulationsParticipating in ongoing education and quality improvement activities Qualifications Active, unencumbered LPN/LVN license in the practicing stateCurrent CPR certification with hands-on componentTB test within the past yearReliable transportation and ability to travelSix months of clinical nursing experience preferred; home health experience desirable Skills & Abilities Attention to detail, organization, and effective communicationProblem-solving and conflict resolution skillsAbility to lift up to 50 pounds and perform physical patient care tasksProficiency in assessment, documentation, and adherence to policies Work environment: Home-based care with exposure to varied conditions, including potential contact with blood and bodily fluids.
