In Home Healthcare LVN - Full Time - Days

We are seeking a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care in a home setting, primarily for pediatric patients with feeding tubes and medication needs. The role involves delivering and documenting patient care, monitoring conditions, and implementing care plans under supervision, while ensuring compliance with regulations and confidentiality standards. Key Responsibilities Include Assessing and responding to patient needs, reporting changes, and documenting activities accurately.Educating patients and families on disease management and self-care techniques.Participating in care planning and updating nursing interventions as needed.Maintaining infection control measures and adhering to safety standards.Providing care based on physician orders and ensuring quality outcomes. Qualifications And Skills Active, unencumbered LVN license in the practicing state.Current CPR certification and TB test within the last 12 months.Six months of clinical nursing experience preferred; home health experience is a plus.Strong attention to detail, time management, problem-solving, and communication skills.Physical ability to lift 50 pounds, reposition patients, and perform essential physical activities.Reliable transportation and ability to work in various environments, including exposure to infectious materials.
