In Home Healthcare LVN - Full Time - Days

About the role

We are seeking compassionate Licensed Vocational Nurses (LVN) to provide skilled nursing care in home settings, primarily for adult patients with conditions such as feeding tubes, medication administration, and non-invasive ventilation.

This role requires adaptability to diverse environments, adherence to safety protocols, and a dedication to compassionate patient care.

Responsibilities

• Deliver and document patient care in accordance with physician orders and care plans.
• Monitor patient conditions, assess needs, and report changes promptly.
• Educate patients and families on disease management, self-care, and safety.
• Maintain compliance with policies, regulations, and safety standards, including infection control.
• Participate in ongoing education and skill development.

Requirements

• Active, unencumbered LPN/LVN license in the practice state.
• Current CPR certification and TB test within the last 12 months.
• Reliable transportation and ability to travel to patient homes.
• Strong assessment, documentation, organization, and communication skills.
• Physical ability to lift, reposition patients, and perform home care tasks.
