In Home Healthcare LVN - Full Time - Days

Responsibilities

• Delivering and documenting patient care according to care plans and physician orders.
• Assessing patient conditions, recognizing changes, and implementing appropriate interventions.
• Educating patients and families on disease management, self-care, and prevention.
• Maintaining confidentiality, adhering to policies, and ensuring infection control standards.
• Participating in ongoing education and skill development.

Requirements

• Active, unencumbered LVN license in the practicing state.
• Current CPR certification and TB test within last 12 months.
• Reliable transportation and relevant clinical experience preferred.

Skills & Abilities

• Attention to detail, effective communication, time management, and problem-solving skills.
• Physical ability to lift up to 50 pounds, reposition patients, and perform various physical tasks.
