About the role
A Licensed Practical/Vocational Nurse (LPN/LVN) provides skilled nursing care in home settings under the supervision of a Registered Nurse.
Responsibilities
- Deliver and document patient care aligned with physician orders and care plans
- Monitor and assess patient conditions, reporting changes promptly
- Maintain accurate records of assessments, interventions, and responses
- Implement nursing actions to achieve desired outcomes and prevent risks
- Educate patients and families on disease management and self-care techniques
- Ensure infection control and compliance with policies, regulations, and standards
Requirements
- Active, unrestricted LPN/LVN license in the practicing state
- Current CPR certification (training available if needed)
- TB skin test within the past 12 months
- Six months of clinical nursing experience preferred
- Reliable transportation and effective communication skills
- Physical ability to lift and reposition patients, and perform necessary movements

- Ability to work in diverse environments and handle exposure to infectious materials
- Participation in continued education and compliance with state-specific requirements
- Physical stamina for prolonged standing, walking, and bending
