In Home Healthcare LVN - Night Shift (High Acuity)

Position: Licensed Practical / Vocational Nurse (LPN/LVN) Responsibilities: Provide compassionate homecare to adult patients, managing care plans that include feeding tubes, trachs, vents, and medication administration (training available). Ensure clinical outcomes through empathetic patient support and documentation using electronic systems. Qualifications: Must hold an active, unencumbered LPN/LVN license in the practice state, with transferable compact licenses within 90 days. Current CPR certification (training available if needed), TB test within last 12 months, and reliable transportation are required. Six months of hands-on nursing experience is preferred but not mandatory. Skills & Attributes: Strong clinical skills, compassionate communication, ability to work independently, and adaptability to flexible schedules including days, nights, weekends, full-time, part-time, or PRN shifts. Ability to collaborate with a multidisciplinary team and utilize electronic documentation tools. Key Duties: Deliver high-quality, patient-centered homecare, follow care protocols, document care activities electronically, and support patients and families with professionalism and empathy.
