In Home Healthcare LVN - Low Acuity - Full Time/Part Time Days

About the role

Join a patient‑focused healthcare team as a Licensed Vocational Nurse (LVN). This role involves providing skilled nursing care to pediatric patients, including feeding tube management and medication administration, under the supervision of a Registered Nurse.

Responsibilities

• Deliver and coordinate quality patient care according to physician orders.
• Assess patient conditions, document activities, and update care plans.
• Recognize and respond to changes in patient needs to ensure safety and optimal outcomes.
• Educate patients and families on disease management, self‑care, and prevention.
• Maintain infection control standards and compliance with regulations.

Qualifications

• Active, unencumbered LPN/LVN license in the practicing state.
• Current CPR certification and TB test within the last 12 months.
• Preferred: at least six months of clinical experience, home health experience, and reliable transportation.
• Strong attention to detail, time management, communication, and problem‑solving skills.

Additional Requirements

• Ability to travel, lift up to 50 pounds, and respond effectively to emergency situations.
• Proficiency in English, physical fitness for patient repositioning, and environmental adaptability.
