About the role
We are seeking a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care in a home setting.

This position requires adaptability across various environments, physical capability, and commitment to safe, quality care.
Responsibilities
- Delivering and documenting patient care following physician orders and care plans.
- Monitoring patient conditions, recognizing changes, and reporting to supervisors or physicians.
- Educating patients and families on disease management, self-care, and prevention.
- Maintaining confidentiality, adhering to safety protocols, and complying with regulations.
- Participating in ongoing education and skill development.
Qualifications and Skills
- Active, unrestricted LPN/LVN license in the practicing state.
- Current CPR certification and TB test within the last 12 months.
- Reliable transportation and ability to perform physical tasks, including lifting and repositioning patients.
- Proficiency in assessments, documentation, organization, and communication.
- Preferred: experience in pediatrics, home health, or recent clinical practice.
- Experience working with pediatric patients with specific acuity needs, such as feeding tubes and medication administration.
