In-Home Healthcare LVN - Low Acuity - Part Time or Full Time

About the role

We are seeking a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care in a home setting, primarily for pediatric patients with feeding tubes and medication needs. The role involves administering treatments, monitoring patient conditions, documenting care, and updating care plans under the supervision of a Registered Nurse. This position is vital for delivering heartfelt, compassionate homecare focused on achieving excellent clinical outcomes.

Responsibilities

• Deliver and coordinate quality patient care per physician orders.
• Assess patient condition and report changes promptly.
• Document assessments, actions, and responses accurately.
• Educate patients and families on disease management and self‑care.
• Maintain infection control standards and adhere to regulatory guidelines.
• Participate in ongoing education to maintain clinical competencies.

Qualifications and Skills

• Active, unencumbered LPN/LVN license in the practicing state.
• Current CPR certification.
• TB test within the past 12 months.
• Pediatric experience preferred.
• Reliable transportation and physical ability to lift up to 50 pounds.
• Strong attention to detail, organization, communication, and problem‑solving skills.
