About the role

We are seeking a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care in a home setting for pediatric patients with complex needs, including feeding tubes and medication administration. The role involves delivering and documenting patient care, assessing conditions, updating care plans, and educating families on disease management and self-care techniques.

Responsibilities

• Provide high-quality, compliant nursing care under supervision of a Registered Nurse
• Monitor and report patient condition changes
• Document assessments, interventions, and outcomes accurately
• Implement infection control measures and adhere to safety protocols
• Educate patients and families to promote health and safety

Qualifications and Skills

• Active, unencumbered LPN/LVN license in the state of practice
• Current CPR certification and TB test within last 12 months
• Reliable transportation and the ability to lift up to 50 pounds
• Strong clinical assessment, documentation, organization, and communication skills
• Preferred: six months of recent pediatric or home health experience

Physical demands and environment

Ability to travel, perform physical tasks including repositioning patients, and work in diverse environments with potential exposure to infectious materials. Must respond effectively to emergency situations.
