In-Home Health - Physician (Full Time) - CT
About the role
Signify Health, part of CVS Health®, is seeking a full-time Physician to provide In-Home Health Evaluations. You’ll engage with people in the comfort of their own homes, helping bridge gaps in care by building trusted relationships and delivering flexible, comprehensive, and personalized health evaluations.
The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. You’ll use an iPad with an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens.
As an in-home clinician, your primary workplace will be the private residences of health plan members. These environments may vary and could include household pets, secondhand smoke, or other airborne allergens. Candidates must be comfortable and safe performing duties in diverse home conditions.
Responsibilities
- Conduct in-home health evaluations, including medication reviews, medical history assessments, and physical evaluations.
- Perform diagnostic and preventive services (e.g., spirometry, diabetic eye exams) as ordered by health plans.
- Travel locally and within licensed states to conduct in-home visits.
- Conduct virtual visits as needed.
- Provide connections to appropriate care based on individuals’ unique needs.
- Document and communicate findings to health plans and primary care physicians.
Requirements
- Active, unrestricted medical license in the coverage area(s).
- Multi-state licenses preferred.
- Able to cover the Hartford, CT area and surrounding regions.

Pay
The typical pay range for this role is $145.00 per completed In-Home Health Evaluation. Pay is dependent on experience, education, geography, and other relevant factors. This position is also eligible for additional reimbursement opportunities, including pay per completed Diagnostic and Preventive Services (DPS), as ordered.
Schedule
- Full-time position with an anticipated 40 hours per week.
