In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Springfield, MA

Remote position open to US-based candidates only; no visa sponsorship is available.

Responsibilities

• Engage with patients in their homes to provide health evaluations
• Build trusted relationships to improve health outcomes
• Conduct medication and medical history reviews
• Perform physical evaluations as needed
• Utilize technology for efficient clinical workflow
• Travel locally within licensed states for in-home visits
• Conduct virtual visits as required

Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification required
• Multi-state licenses preferred
• Ability to cover primary locations: Springfield MA - Hampden County
• Experience in patient engagement and health evaluations
