In-Home Health Care RN - High Acuity Days - Immediate Weekends Available

About the role

We are seeking compassionate Registered Nurses (RNs) to provide skilled home healthcare for adult patients with complex needs, including feeding tubes, medication administration, tracheostomy, and ventilator support.

Responsibilities

• Delivering and documenting personalized nursing care in accordance with physician orders and care plans.
• Assessing patient conditions, monitoring changes, and reporting to supervisors or physicians.
• Implementing appropriate nursing interventions to achieve clinical outcomes and prevent risks.
• Educating patients and families on health management, disease processes, and self-care techniques.
• Maintaining compliance with regulatory standards, infection control, and confidentiality regulations.
• Developing, updating, and coordinating nursing care plans based on ongoing assessments.

Qualifications

• Active, unrestricted RN license in the practice state.
• Current CPR certification and TB testing within 12 months.
• Reliable transportation and clinical assessment skills.

Preferred: Six months of recent clinical experience; home health experience is a plus.

Skills

Required skills: attention to detail, effective communication, time management, problem-solving, and adaptability in diverse environments. Physical ability to lift up to 50 pounds and perform home care duties is necessary.
