In-Home Health Care RN - High Acuity Days - Immediate Weekends Available

About the role

We are seeking compassionate Registered Nurses (RNs) to provide skilled home healthcare for adult patients with complex needs, including feeding tubes, tracheostomy, and ventilator support.

Responsibilities

• Deliver and document personalized nursing care in accordance with care plans and physician orders.
• Assess patient conditions, recognize changes, and implement appropriate interventions.
• Develop, update, and execute nursing care plans to achieve optimal outcomes.
• Educate patients and families on disease management, self‑care, and prevention strategies.
• Maintain compliance with healthcare regulations, infection control standards, and confidentiality.

Qualifications and Skills

• Active, unencumbered RN license in the state of practice (compact licenses accepted if transferred within 90 days).
• Current CPR certification and TB test within the last 12 months.
• Minimum six months of hands‑on nursing experience preferred.
• Reliable transportation and effective communication skills.
• Proficiency in clinical assessments, documentation, and compliance with policies.

Additional Requirements

• Ability to perform physical tasks such as lifting up to 50 pounds, prolonged walking, and repositioning patients.
• Willingness to function in diverse environments, potentially exposing to blood and infectious materials.
