In-Home Health Care RN - High Acuity Days - Immediate Weekends Available

About the role

We are seeking a compassionate Registered Nurse (RN) to provide skilled home healthcare for adult patients with complex needs, including feeding tubes, medication administration, tracheostomy, and ventilator support.

Responsibilities

• Delivering and documenting patient care per physician care plans and assessments.
• Monitoring patient conditions, reporting changes, and implementing appropriate interventions.
• Developing, updating, and executing nursing care plans to achieve optimal outcomes.
• Educating patients and families on disease management, self-care, and prevention.
• Ensuring compliance with healthcare standards, infection control, and confidentiality protocols.

Qualifications

• Active, unencumbered RN license in the practice state.
• Current CPR certification and TB screening.
• Proficiency in clinical assessments, documentation, and adherence to policies.
• Reliable transportation and physical ability to lift up to 50 pounds, stand, walk, and respond to emergencies.
• Effective communication, organization, and problem-solving skills.

Skills

Preferred experience: Six months of recent clinical or home health experience; specific discipline experience (e.g., oncology, surgical, ambulatory) is advantageous.
