In-Home Health Assessment Provider (PRN)

About the role

We are seeking a Nurse Practitioner (NP) to perform comprehensive in-home health assessments for adult and geriatric patients in their residences or skilled nursing facilities.

Responsibilities

• Conduct medical history reviews, health assessments, and social/environmental evaluations during visits up to one hour
• Administer and order point-of-care and lab tests
• Collaborate with physicians, case managers, social workers, and family members
• Provide patient education, follow-up, and documentation per protocols
• Support care management to improve health outcomes and address care gaps

Requirements

• Master’s Degree or equivalent, with current NP licensure and prescriptive authority
• Board certification in Adult, Gerontology, Family, or Acute Care
• Current BLS, ACLS, or CPR certification
• At least 1 year of NP experience preferred; new graduates encouraged
• Valid driver’s license, automobile insurance, and computer proficiency
