Role Overview

This position involves conducting comprehensive health assessments for adult and geriatric members in their homes or facilities, reviewing medical history, medications, social factors, and health risks to improve care quality and address care gaps.

Key Responsibilities

  • Perform assessments up to one hour, documenting findings per protocols.
  • Administer and order point-of-care and lab tests.
  • Collaborate with physicians, case managers, social workers, and caregivers.
  • Provide patient education and follow-up in coordination with primary care providers.
  • Visit members at their residences, including homes and skilled nursing facilities.

Qualifications & Skills

  • Master’s degree or equivalent with NP licensure, certification (AANP, AACN, ANCC).
  • Current BLS/ACLS/CPR certification.
  • Experience preferred but new graduates encouraged.
  • Valid driver’s license, automobile insurance, and computer proficiency.
  • Excellent communication skills.
