About the role
Seeking a Nurse Practitioner to conduct comprehensive in-home health assessments for adult and geriatric patients.
Responsibilities
- Performing assessments covering medical history, diagnoses, treatments, and social factors
- Administering and ordering point-of-care and lab tests
- Collaborating with physicians, case managers, social workers, and caregivers
- Providing health education, referrals, follow-up, and documentation
- Delivering services at members' residences, including homes and skilled nursing facilities
Qualifications
- Master's Degree or equivalent, with current NP licensure and prescriptive authority
- Board certification in Adult, Gerontology, Family, Emergency, or Acute Care specialties
- Current BLS, ACLS, or CPR certification
- Valid driver's license and auto insurance
- Strong communication, computer skills, and ability to work independently
