In-Home Health Assessment Provider (PRN)

Responsibilities

  • Performing adult and geriatric health assessments
  • Reviewing medical history, diagnoses, and treatment plans
  • Administering and ordering point-of-care and lab tests
  • Collaborating with physicians, case managers, social workers, and caregivers
  • Providing health education, referrals, follow-up, and documentation according to protocols
  • Working independently with flexible scheduling and travel to member locations

Qualifications And Skills

  • Master’s Degree or equivalent experience with NP licensure and prescriptive authority
  • Board certification in adult, gerontology, family, emergency, or acute care
  • Current BLS, ACLS, or CPR certification
  • Valid driver’s license, automobile insurance, and strong computer skills
  • Excellent communication skills; experience preferred but new grads encouraged
