In-Home Health Assessment Provider (PRN)

About the role

Provide comprehensive in-home health assessments to adult and geriatric members, reviewing medical history, medication adherence, social environment, and health risks to improve care quality and identify undiagnosed conditions.

Responsibilities

• Conduct assessments, including medical history, diagnosis, treatment, and health education
• Administer point-of-care and lab tests as needed
• Collaborate with physicians, case managers, social workers, caregivers, and other medical personnel
• Coordinate follow-up and provide patient education in members’ homes or facilities

Qualifications

• Master’s Degree or equivalent with NP licensure and prescriptive authority
• Board certification in relevant adult or family care specialties
• Current BLS, ACLS, or CPR certification
• Valid driver’s license and automobile insurance
