About the role

The ICMS Case Manager provides coordinated case management and supportive services for formerly homeless individuals diagnosed with special needs, delivering services on-site at a newly developed multi-unit permanent supportive housing complex.

Responsibilities

  • Conduct initial housing intake and needs assessment for each tenant.
  • Assist tenants in determining personal goals and developing Individual Service Plans (ISP).
  • Address pre-existing conditions and obstacles to facilitate timely completion of housing-related processes.
  • Conduct a minimum of two face-to-face contacts with each tenant monthly.
  • Hold quarterly meetings to discuss tenant progress and accomplishments as outlined in Individualized Stability Plans.
  • Act as a liaison among the Property Manager, property management company, tenant, and primary mental health provider to address concerns that may jeopardize tenancy.
  • Provide information and instruction on Fair Housing, tenant rights and responsibilities, rental agreements, and property rules.
  • Coordinate linkage to services with public social service agencies and community providers, including employment, education, money management, legal, and benefits counseling.
  • Assist clients in obtaining rental assistance, move-in services, and eviction prevention.
  • Address tenant grievances, mediate resident disputes, and resolve issues with property management affecting tenancy.
  • Enforce housing rules in collaboration with property management.
  • Work with the Resident Services Coordinator to develop workshops on housekeeping, money management, parenting, daily living skills, and self-advocacy.
  • Document client progress, compile reports, and evaluate programs using DHS CHAMP’s database.
  • Manage client files and monitor tenants transitioning to mainstream non-supportive housing.
  • Comply with documentation and reporting requirements as per the service contract.
  • Compile and submit monthly progress reports tracking residency status and supportive services provided.
  • Uphold CRCD’s Mission Statement and 5-Year Strategic Plan.

Requirements

  • 1-2 years of experience or related work and a BA Degree.
  • Experience working with youth experiencing homelessness, housing instability, foster care, probation, or mental health systems.
  • Reliable, flexible team player with strong work ethic and ability to work with minimal supervision.
  • Reliable transportation: valid driver’s license and car insurance as required by law.
  • Proficient in Microsoft Office suite.
  • This is an in-office role, reporting to the office 5 days a week.

Pay

$50,000.00 - $55,000.00 annually.
