Essential Duties and Responsibilities

  • Completes an individualized client needs assessment on cases referred for services to determine initial and ongoing eligibility.
  • Service plan development and execution; develops outcomes, manages multiple providers to support outcomes, and monitors progress towards meeting outcomes, and completes authorizations for purchased services and/or supports.
  • Provides case management and coordinative services to assist clients in gaining and retaining access to services necessary to maximize their level of functioning.
  • Shares community resources.
  • Works closely with immediate family members of the client to assist them in understanding, accepting and implementing the service plan for the client.
  • Promotes child's health, safety and welfare in the home and community.
  • Cross-system coordination; advocacy/collaboration for coordination with other services and resources.
  • Refers or coordinates client case plan with other agencies providing services to meet client needs.
  • Assists other Agency staff in determining appropriateness of specialized services to supplement a client case plan.
  • Will work to locate and assists in the development, recruitment, licensing or certification of resources necessary to implement case plans, as needed.
  • May follow up on court-ordered services, if needed.
  • Assists in short- and long-range planning, developing and identifying needed resources for long-term support populations.
  • Provides community education/consultation/intervention regarding Department services.
  • May develop ideas and create educational posts for social media.
  • Transitional support: discusses legal authority changes when turning 18 and assists in the transitioning to adult programming, as needed.
  • Maintains case records containing pertinent, accurate and current information; prepares correspondence, reports and other records as required.
  • Shall be assigned to perform crisis on-call duty.
  • Works a flexible schedule to meet the needs of the families.

Supervisory Responsibilities

  • Provides direction to professional and para-professional staff.

Qualifications

  • Ability to exercise judgment and discretion in the application and interpretation of Department policies and regulations.
  • Knowledge of human growth and behavior; specific range of treatment modalities peculiar to the professional specialty area of the employee; social casework principles and methods; and community based human services delivery system and resources available in the community.
  • Ability to satisfactorily perform the following functions or demonstrate the following skills including: establish a therapeutic relationship with clients; make independent decisions; establish and maintain effective working relationships with clients, co-workers and other agencies and the public; and provide complex services in accordance with principles common to the specialty of the employee.
  • Ability to apply laws of confidentiality appropriately.
  • Demonstrates the ability to interpret, support and uphold the standards, ethics and boundaries of the social work profession.
