Housing | Case Navigator

About the role

Catholic Charities is seeking a dedicated Case Navigator to join our team in the Houston-Galveston area. This position offers a rewarding career with a supportive family-friendly organization that helps people facing homelessness.

Responsibilities

• Assess clients' needs and coordinate supportive services
• Assist clients in accessing housing resources
• Utilize the EMPath matrix to assess clients
• Connect clients to internal and external resources for rent and utility assistance, employment services, food, counseling, and benefits
• Record data and maintain up-to-date physical files using the Homeless Management Information System
• Conduct eligibility screenings to understand unique needs and challenges
• Identify strengths and barriers to housing stability and connect to appropriate resources
• Support clients in establishing and meeting housing and related goals
• Prepare reports and data analysis to track program progress and outcomes
• Offer support and guidance to help clients overcome obstacles and maintain housing stability
• Handle crisis situations and connect clients to appropriate resources and services
• Perform home visits and inspections to ensure units meet HUD housing standards (NSPIRE)
• Carry a caseload of families and individuals, meeting with them twice a month to check on goal progress and make necessary adjustments
• Document case management services accurately and maintain precise records
• Perform other duties as assigned

Qualifications

• Bachelor degree in social work, psychology, or related field required
• Two (2) years of case management experience within homelessness services, housing programs, or social service
• One (1) year of case management experience preferred
• NSPIRE Inspector Training (provided by agency- must pass within first 90 days of employment)
• Valid Texas Driver’s License

• Knowledge and experience of housing issues and available resources
• Knowledge of EMPath case management model
• Experience and knowledge with the challenges of and resources for those at-risk of homelessness
• Ability to work with diverse communities
• Excellent organizational skills and self-motivation
• Excellent written/verbal communication and people skills
• Intermediate MS Office skills: including Word, Outlook, and using the internet
• Ability to work under pressure, address multiple priorities, and meet deadlines
• Ability to identify key issues in problem-solving and implement effective and creative solutions
• Knowledge of housing resources, social services, and community resources

Pay

Negotiable based on experience and qualifications.
