About the role
The Housing Case Manager position at Access Services in Fort Washington, PA combines expertise in mental health and housing stability. This dual-role position supports individuals in Montgomery County, PA on their path to lasting stability and wellness.
Responsibilities
- Meet with referred individuals and new tenants, coordinating with the Housing Locator and Access Services staff.
- Support individuals in creating and updating Housing Stability Plans, meeting every three months.
- Maintain organized documentation for move-in and recertification requirements, and ongoing documentation for all individuals served.
- Conduct monthly home visits and support during transitions, ensuring transportation for essential activities.
- Identify and address barriers to housing stability with innovative solutions, connecting individuals to social, health, and benefit resources.
- Ensure compliance with HUD requirements and funding guidelines, participating in county and consumer meetings.
- Develop and update Individual Rehabilitation Plans (IRPs) for individuals in the Mobile Psychiatric Rehabilitation program, collaborating with the team to achieve personal goals.
- Prepare case review documents and contribute to team discussions, maintaining accurate financial records.
- Maintain open communication with supervisors and participate in weekly supervision, collaborating with external agencies and professionals.
- Actively assist in resolving crisis situations, ensuring the highest quality of care.
Qualifications
- Education: Bachelor's degree in a related field such as human services, social work, psychology, or special education, or Certified Peer Specialist certification.
- Certification: Completion of Certified Psychiatric Rehabilitation Practitioner certification and department-approved twelve-hour psychiatric rehabilitation orientation course within 12 months of hire.
- Experience: One year of relevant experience in housing support or direct supportive services, with preference given to experience in the housing system in Montgomery County and working with adults with mental illness.
Skills and Abilities
- Strong interpersonal skills, effective communication, and the ability to prioritize work and collaborate with a team.
- Knowledge of the mental health system, homelessness, and the housing system, with the ability to deepen understanding to provide excellent service.
- Person-centered screening and assessment skills, conflict and crisis resolution abilities, and proficiency in workplace applications and technologies.
Requirements
- Valid US driver’s license for at least two years, not including time with permit.
- Able to drive a reliable vehicle during work hours, with valid registration and inspection stickers, valid auto insurance, and proof of being a covered driver on the policy.
