Housing Case Manager - HSP
Starting pay range: $24.59–$26.32 per hour.
RADIAS Health is hiring two Housing Case Managers—one serving Hennepin County and one serving Ramsey County—to provide professional, quality program services to clients in the Housing Support Program (HSP).
About the role
The Housing Case Manager supports individuals with disabilities experiencing homelessness to help them live independently in the community. They assist clients in locating, securing, and maintaining independent housing; provide instruction and monitoring of basic living and social skills; and help restore skills essential for managing illness, treatment, and everyday independent living.
This is a hybrid role with time spent meeting clients in the community and flexibility to work from home or the office for administrative responsibilities.
Hours: Monday–Friday, 8 am–4 pm or 9 am–5 pm, 40 hours per week.

Responsibilities
- Provide Housing Support Services to eligible clients in compliance with County, State, and Medicaid requirements.
- Use person-centered approaches to assist clients in obtaining and maintaining housing, moving, and securing necessary furnishings.
- Assist clients with applying, maintaining, and understanding benefits such as Cash, SNAP, HSS eligibility, medical insurance, and Social Security. Help with completing paperwork, obtaining proofs, getting documents signed by Qualified Professionals, contacting relevant County offices, setting up appointments, and referring to outside resources.
- Provide assistance in budgeting and use of Housing Support funds as required by DHS.
- Assist, teach, and support clients with activities of basic independent living skills, being a good tenant, and understanding their rights as renters.
- Find and assist clients with accessing community resources.
- Communicate effectively and professionally with property management, county financial workers, other providers, and community members to build relationships, advocate for clients, and provide tenant support and mediation.
- Maintain records of progress for all direct and collateral contact, including assessments of clients’ ability to function and manage symptoms.
- Assist clients in creating and updating service plans to work toward goals in regular meetings.
- Provide instruction and monitoring on identified service areas in the client’s Professional Statement of Need and/or Housing Focused Person-Centered Plan.
- Monitor client progress, coordinate with the service team, and modify service plans as appropriate.
- Communicate changes in service needs and recommendations to the Program Director, Mental Health Professional, Case Manager, and service team to coordinate care.
- Schedule and conduct client appointments in their home or community, adjusting frequency and duration based on client needs.
- Intervene in emergencies when a client is unable to manage their illness, contacting appropriate emergency services, doctors, staff, and emergency contacts.
- Coordinate with other agency and community providers, including landlords and resources, to ensure the best possible care for clients. Complete and maintain current release of information documents.
- Maintain compliance with applicable Minnesota Statutes or rules, including DHS Maltreatment of Vulnerable Adults, Duty to Warn, Minnesota Data Practices Act, and HIPAA.
- Meet regularly with the Program Director for individual and group reviews of client needs and receive direction for treatment.
- Transport clients in the community as needed.
Requirements
- Bachelor’s degree in Psychology or Social Work plus 2,000 hours of work experience providing services to adults diagnosed with a serious mental illness, or
- No college degree and 4,000 hours of supervised work experience in the delivery of services to adults or children with mental illness.
- Must have a car, auto insurance, and a valid driver’s license.
