Housing Case Manager

Catholic Charities of Orange, Sullivan & Ulster is seeking a full-time Housing Case Manager for their Sullivan County Case Management program. The hourly rate is $25.00/hour, and this position includes benefits.

About the role

The Housing Case Manager will work directly with Permanent Supportive Housing clients, their immediate family members, and clinical staff to assess client needs and ensure coordinated care. The role involves collaborating with landlords and community providers to assist clients in accessing apartments and available resources.

Responsibilities

  • Performs quarterly service plan updates and progress reviews per policy and regulatory requirements.
  • Maintains case record documentation in accordance with program policy and procedure, including paper documents such as leases and paystubs.
  • Identifies adequate and affordable housing for clients.
  • Conducts HQS inspections for initial and annual apartments as requested by the Sullivan County Supervisor.
  • Maintains client confidentiality in accordance with agency policy, 42 CFR, Part 309, and 14 NYCRR, including obtaining new Release of Information yearly.
  • Coordinates client care with the case management department, including attending treatment-planning conferences as requested.
  • Monitors residences for cleanliness and safety.
  • Maintains regular contact with housing clients through phone, email, text, one home visit per month, and one office visit, ensuring compliance with program rules.
  • Participates in Catholic Charities events such as Stuff the Bus, Adopt A Family, Toy Drive, Food Pantry Distributions, and other activities as needed.
  • Coordinates client referrals and follow-up to community resources, such as vocational/educational and childcare services.
  • Monitors residents’ ability to live independently, including daily living skills, community resource utilization, and self-help engagement.
  • Monitors resident behavior to ensure compliance with program rules and intervenes as needed.
  • Attends staff meetings and in-service training as requested.
  • Maintains current certifications by attending relevant workshops and training.
  • Performs other duties as requested.

Schedule

This is a full-time position with standard working hours of 9:00 AM to 5:00 PM, Monday through Friday. Additional hours may be required to meet program deadlines, client needs, or to attend events.

Working Conditions

  • Ability to work with the public and/or under potentially stressful conditions, including understanding client engagement and de-escalation techniques.
  • Prepare comprehensive progress notes in a timely fashion.
  • Mobility to various apartment sites within the county to accommodate client schedules.
  • Must be able to stand for long periods, safely enter and exit buildings and client homes in various weather conditions, and lift at least 25 pounds.

Requirements

  • GED or High School Diploma (required).
  • Associate’s or Bachelor’s Degree in a related field (preferred).
  • CASAC or CASAC-T certification (desirable).
  • At least one year of experience working with clients with mental health disorders and/or recovering from chemical dependency.
  • Must possess and maintain a clean NYS Driver’s License throughout employment and have reliable transportation.
  • Effective oral, written, and computer skills.
  • Strong interpersonal skills for working with clients and community liaisons.

Pay

$25.00/hour.
