About the Role

As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will conduct in-home health assessments for Medicare Advantage and other plan members, focusing on preventive care and health education. You will review medical history, perform physical exams, screenings, and point-of-care tests, identify diagnoses, and communicate findings to primary care providers. You will recognize urgent situations, provide patient education on disease management, medications, and social determinants of health, and offer necessary referrals.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plan members
• Review medical history and perform physical exams, screenings, and point-of-care tests
• Identify diagnoses and communicate findings to primary care providers
• Recognize urgent care situations and provide appropriate interventions
• Educate patients on disease management, medications, and social determinants of health
• Provide necessary referrals for further care or support
• Navigate varied home environments to perform assessments

Requirements

• Active, unrestricted licensure and certification (ANCC/AANP for NPs, NCCPA for PAs)
• Valid driver’s license and reliable transportation
• Ability to navigate home environments and perform assessments in varied settings
• Physical ability to transport equipment

Preferred Qualifications

• 1+ years of clinical experience
• Proficiency with electronic medical records (EMRs)
• Strong communication skills
