About the Role

As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will perform comprehensive in-home health assessments for Medicare Advantage members, focusing on improving health outcomes through education, gap closure, and collaboration with primary care providers.

Responsibilities

• Conduct medical history reviews, physical exams, vital signs, and screenings in patients' homes.
• Document diagnoses for care management and communicate findings to primary care providers (PCPs).
• Identify urgent situations and intervene appropriately.
• Educate members on health, medications, and social determinants, providing referrals as needed.

Requirements

• Active certification (ANCC/AANP for NPs; NCCPA for PAs) and unrestricted state licensure.
• Valid driver’s license, reliable transportation, and ability to navigate home environments.

Preferred Qualifications

• 1+ years of clinical experience.
• Proficiency with electronic health records.
• Strong communication skills.
