About the Role

As a HouseCalls Advanced Practice Clinician, you'll perform in-home health assessments for Medicare Advantage and other plan members, focusing on preventive care, education, and care gap closure. This field-based position emphasizes collaboration with primary care providers to improve health outcomes without prescribing medications.

Responsibilities

• Conduct comprehensive assessments, including medical history, vital signs, physical exams, screenings, and point-of-care tests
• Identify and document diagnoses for care management
• Communicate findings to primary care providers (PCPs)
• Recognize urgent situations and intervene appropriately
• Educate members on disease management and medication adherence
• Address social determinants of health and provide referrals

Requirements

• Active licensure (NP or PA) in the practicing state
• Certification from ANCC, AANP, or NCCPA
• Unrestricted driver’s license and reliable transportation
• Ability to navigate diverse home environments and perform physical assessments

Preferred Qualifications

• 1+ years of clinical experience
• Proficiency with EMRs
• Strong communication skills with older or Medicare populations

Schedule

Flexible scheduling (8am–8pm), with no on-call, weekends, or holidays.
