About the Role

We are seeking a dedicated and clinically astute Nurse Practitioner to join our geriatric clinical team. In this role, you will serve as the primary clinical presence within assigned Skilled Nursing Facilities (SNFs), bridging the gap between acute hospital discharge and long-term recovery.

Pay

$108,466.00 to $162,698.00 (anticipated base salary range; actual compensation determined by experience, skills, qualifications, geographic location, internal equity, and business needs)
