An advocate for the patient, the Advanced Practice Provider (APP) delivers comprehensive clinical management in a home setting, collaborating with the House Calls Medical Director and leadership team to ensure timely, patient-centered, evidence-based care.

Responsibilities

  • Provides clinical medical management to meet patient needs and prevent unnecessary Emergency Department utilization when appropriate.
  • Delivers in-home services including IV hydration/antibiotic therapy, nebulizer treatments, wound care, injections, and mobile diagnostics as indicated.
  • Participates in the on-call rotation, providing after-hours telephonic support to patients (approximately 6-10 shifts/year).
  • Documents all pertinent and active medical conditions in the electronic medical record in a timely, compliant manner per MPG policy, CMS, and other insurance carriers.
  • Addresses outstanding Quality Measures and HCCs to meet or exceed all quality goals.
  • Completes all documentation and billing in a timely manner, per protocol.
  • Exhibits strong clinical skills to improve care for an aging, adult population and communicates health care information effectively to patients and families.
  • Identifies patients needing additional assistance and makes referrals to home health, social services, behavioral health, skilled nursing, and other community resources as needed.
  • Attends and participates in interdisciplinary meetings to improve patient clinical outcomes.
  • Performs other duties as assigned.

Requirements

  • Completion of a master's degree in nursing.
  • Certification as an Advanced Registered Nurse Practitioner.
  • Knowledge and experience with documentation in electronic health records and healthcare-related communication platforms.
  • Demonstrated competencies in compassion, compliance, communication, dependability, integrity, and patient service orientation.

Physical Demands

  • Regularly required to stand, walk, use hands to finger/handle, reach with hands and arms, climb or balance, and talk or hear.
  • Frequently required to sit.
  • Occasionally required to stoop, kneel, crouch, or crawl.
  • Must occasionally lift and/or move up to 10 pounds.
  • Specific vision abilities include close, distance, color, peripheral, depth perception, and ability to adjust focus.

Work Environment

  • In-home patient visits are required, exposing the employee to outside weather conditions and varied patient home environments.

Pay

Anticipated base salary range: $103,043.00 to $154,563.00. Actual compensation will be determined based on experience, skills, qualifications, geographic location, internal equity, and business needs.
