Host Home Provider - Wheelchair Accessible

HHP Provider Duties And Responsibilities

• Provide personal cares including assisting with hygiene care, meal preparation, dressing
• Provide employment coaching to assist in getting a job
• Provide a variety of recreational activities to enhance social development
• Include client in family life and activities
• Accompany client to supermarket, museums, parks, dances, recreation centers, etc.
• Provide transportation as needed
• Assist with medical care, including appointments and administering medications
• Provide documentation of progress notes and medical records
• Train on life learning skills

Requirements

• High school or equivalent education
• Complete and pass criminal and motor vehicle background checks
• Complete training for Medication Administration, CPR/First Aid and maintain ongoing training requirements
• At least 1 year of experience as a host home provider
• Wheelchair accessible home
• Experience with behavioral needs preferred
• Experience with medical needs preferred (CNAs, LPNs, RNs, etc.)

Qualifications

• Should have a High school or equivalent education
• Should preferably be experienced with behavioral needs
• Should preferably be experienced with medical needs, preferred (i.e. CNAs, LPNs, RNs, etc.)

Pay

Salary Range: $18,000.00 to $65,000.00/year depending on the clients’ disability level
