Host Home Provider

About the role

We are hiring for Host Home Providers at RHA Health Services. This is a Regular position. RHA Health Services is an industry leader offering comprehensive, person-centered services for individuals with intellectual and developmental disabilities (I/DD).

Responsibilities

• Understand the individual in the context of their personal history, social and family networks, and hopes and dreams for the future.
• Support the individual in eating nutritious meals and snacks consistent with dietary needs, resting, daily self-care activities, leisure/recreation, education, and working.
• Ensure privacy and dignity while supporting the individual's choices, preferences, and culture.
• Prepare, teach, and assist in food preparation and cleaning of the home.
• Honor the choices, preferences, and culture of the person you support.
• Aid in meeting the social, emotional, and spiritual needs of the person you support.
• Assist with transportation and scheduling of all medical, therapy, vocational, and recreational activities.
• Ensure the home environment is safe and in good condition.
• Implement services and supports to address goals identified through each person’s Person Centered Plan, including medical, psychiatric, recreational, spiritual, and emotional needs.
• Attend treatment team meetings and facilitate communication related to individual needs, response to service and supports, and progress toward goals.
• Participate in supervision as agreed upon in the RHA supervision contract.
• Complete the daily documentation of services for billing in the electronic record system.
• Maintain training compliance such as CPR, Safety Care, as outlined by DBHDD.
• Maintain equipment (Laptop, phone, etc.).

Qualifications

• High School Diploma/GED preferred
• Must be 18 years old or older
• Documentation of home ownership: current mortgage statement or renter’s lease
• Proof of homeowner’s or renter’s insurance or personal property insurance
• Complete all Host Home Study requirements including training
• Valid Driver’s License; Reliable transportation with valid vehicle insurance
• Ability to pass criminal background and Registry checks as well as a Drug Screen
• Proof of employment history or references (if required)
• Positions that require driving: Proof of driver's license, driver's insurance, and vehicle
• Pre-employment screening: Complete criminal background, Name checked in the registries (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
• Drug testing
• Education verification and other credentialing based on position requirements
• Complete all necessary training and certifications
