Hospice RN On Call (Weekend Day)

About the Role

On-call RN positions available with the following schedule: Friday 10a–10p, Saturday and Sunday 8a–8p.

Responsibilities

  • Assume responsibility for patient visits or on-call schedule, including assessing, planning, implementing, and evaluating phases of the nursing process.
  • Provide hospice nursing care based on systematic assessment focusing on pain control and symptom management.
  • Initiate communication with attending physicians, other hospice staff members, and agencies to coordinate care and use resources for the patient/family.
  • Maintain regular communication with direct supervisor to review assignments.
  • Maintain up-to-date documentation at the point of service, accurately reflecting problems, plans, actions, goals, and charges in accordance with agency policy and Medicare regulations.
  • Administer medication in compliance with federal and state laws and hospice policies, alongside long-term care staff.
  • Maintain current patient records and documents to ensure accurate reflection of problems, interventions, goals, outcomes, and charges.
  • Assess and collaborate with the Medical Director to determine initial and ongoing eligibility of hospice patients.
  • Complete an initial assessment of patient and family to determine home care needs.
  • Anticipate end-stage disease processes and related pain and symptom management.
  • Regularly re-evaluate the patient’s nursing needs.
  • Use assessment data to determine care plan needs.
  • Initiate appropriate preventive and palliative nursing procedures.
  • Counsel the patient and family in meeting end-of-life needs.
  • Perform a comprehensive death note when appropriate.
  • Communicate with physicians regarding patient needs and report changes in patient condition.
  • Coordinate with other members of the interdisciplinary team (IDT) to develop and implement the care plan.
  • Participate in on-call duties as defined by the on-call policy.
  • Ensure arrangements for necessary equipment, items, and services are made.

Requirements

  • Associate’s Degree in Nursing.
  • State Certified RN license.
  • Experience in either hospice or home health care (required).
  • Excellent documentation skills and ability to work independently.
  • Strong communication skills.
  • Valid driver’s license and reliable transportation.
  • Must be available to travel locally within assigned territory.
