Hospice RN - Weekend On-Call

About the Role

We are seeking a dedicated Registered Nurse (RN) to join our Hospice division, providing vital patient care during weekends. This position offers flexibility, allowing nurses to advocate for patients, collaborate with interdisciplinary teams, and support symptom management.

Responsibilities

• Conduct ongoing assessments of patients' physical and psycho-social status.
• Develop and update individualized care plans in collaboration with the team, patients, and families.
• Provide education and interventions for pain management and symptom control.
• Ensure availability of equipment, supplies, and medications.
• Document patient services accurately and timely.
• Participate in interdisciplinary team meetings.

Requirements

• Active, unrestricted RN license in Michigan.
• Minimum of one year of relevant RN experience; home health and management experience preferred.
• Current CPR certification.
• Reliable transportation, valid driver’s license, and insurance.
