About the role
Provide patient-centered hospice care and support within a flexible, weekend-only schedule.
Responsibilities
- Conduct ongoing assessments of patients' physical and psycho-social status
- Develop and adjust care plans collaboratively with the Interdisciplinary Group, patients, families, and physicians
- Educate patients and families, manage pain and symptoms, and evaluate responses to treatments
- Ensure availability of necessary equipment, supplies, and medications
- Maintain accurate documentation and participate in IDG meetings
Requirements
- Active, unrestricted RN license in Michigan
- Minimum of one year of relevant RN experience
- CPR certification
- Reliable transportation, valid driver’s license, and insurance
Preferred Qualifications
- Management and home health experience
- Medical-Surgical background
