Hospice RN On Call Weekend - Days

Paradigm Health is seeking a full-time Hospice RN for on-call shifts. Schedule

• Friday: 5:00 PM – 11:00 PM
• Saturday/Sunday: 8:00 AM – 11:00 PM

On-call responses are required within one hour for assigned territory needs.

Responsibilities

• Perform physical and psychosocial assessments on all hospice patients.
• Adequately assess and reassess pain, utilizing appropriate pain management techniques.
• Educate patients and families regarding pain management.
• Respond to the physical, mental, social, and emotional needs of terminally ill patients and their families.
• Assess patient’s condition, initiate plan of care, and re-evaluate/update as necessary.
• Obtain all necessary physician orders and collaborate with physicians as patient needs dictate.
• Initiate case conferences to discuss multidisciplinary team responsibilities, pain management, and bereavement counseling.

Requirements

• One year of hospice experience required.
• One year of RN experience required.
• Licensed Registered Nurse in the State of Indiana.
• Current driver’s license.
• Reliable transportation and insurance.
