About the role
The RN-On Call Nurse provides hospice nursing care on a Friday 5:00 PM to Monday 8:30 AM schedule, focusing on patient assessment, planning, implementation, and evaluation.
Responsibilities
- Assumes responsibility for patient visits or on-call schedule, including assessing, planning, implementing, and evaluating phases of the nursing process.
- Provides hospice nursing care based on systematic assessment, focusing on pain control and symptom management.
- Initiates communication with attending physicians, other hospice staff, and agencies to coordinate care and use resources effectively.
- Maintains regular communication with direct supervisor to review assignments.
- Ensures up-to-date documentation at the point of service, accurately reflecting problems, plans, actions, goals, and charges in accordance with agency policy and Medicare regulations.
- Administers medication in compliance with federal and state laws and hospice policies.
- Maintains current patient records and documents per Ascend Hospice & Palliative Care MA policies.
- Assesses and collaborates with the Medical Director to determine initial and ongoing eligibility of hospice patients.
- Completes initial assessments of patients and families to determine home care needs.
- Anticipates end-stage disease processes and related pain and symptom management.
- Regularly re-evaluates patient nursing needs and uses assessment data to determine care plan needs.
- Initiates appropriate preventive and palliative nursing procedures.
- Counsels patients and families in meeting end-of-life needs.
- Performs comprehensive death notes when appropriate.
- Communicates with physicians regarding patient needs and reports changes in patient condition.
- Coordinates with members of the Interdisciplinary Team (IDT) to ensure a cohesive care plan.
- Participates in on-call duties as defined by the on-call policy.
- Ensures arrangements for necessary equipment, items, and services are made.
- Must be available to travel locally within the assigned territory.
Requirements
- Associate's Degree in Nursing.
- State Certified RN license.
- Experience in hospice or home health care (required).
- Excellent documentation skills and ability to work independently.
- Strong communication skills.
- Valid driver’s license and reliable transportation.
