Hospice RN On-Call

About the role

Provide home-based care to patients with limited life expectancy, focusing on quality of life. Collaborate with an interdisciplinary team including physicians, LPNs, CNAs, social workers, chaplains, and volunteers to deliver holistic care addressing physical, emotional, and spiritual needs.

Responsibilities

• Perform patient assessments and monitor vitals
• Manage pain and administer medications
• Ensure medical equipment and supplies are accessible
• Educate families on prognosis, care plans, and hospice goals
• Conduct patient assessments and monitor health status
• Manage medications and treatments
• Educate families to ensure comfort and support during end-of-life care
• Promote patient dignity and independence in a home environment

Requirements

• Active, unrestricted RN license in Michigan
• Minimum of one year of relevant RN experience, preferably in community health or Medical-Surgical care
• Reliable transportation, valid driver’s license, and current insurance
