Hospice RN On-Call

About the role

Provide compassionate, home-based care to patients with limited life expectancy, focusing on quality of life and holistic support.

Responsibilities

• Conduct patient assessments, monitor vitals, manage pain, and administer treatments.
• Collaborate with an interdisciplinary team to implement the patient's care plan.
• Educate families about prognosis, medications, and hospice goals.
• Ensure medical supplies and equipment accessibility.
• Deliver personalized end-of-life care, support patients and families, and work autonomously within a team to improve patient comfort and dignity.

Requirements

• Active, unrestricted RN license in the state.
• Minimum of one year of relevant RN experience, preferably in community health or Medical-Surgical settings.
• Reliable transportation, valid driver’s license, and current insurance.
