About the Role
Provide home-based care to patients with a prognosis of six months or less, focusing on improving quality of life. Take a holistic approach addressing physical, emotional, spiritual, and family needs in a collaborative, interdisciplinary setting.
Responsibilities
- Collaborate with an interdisciplinary team including physicians, LPNs, CNAs, social workers, chaplains, and volunteers to execute care plans.
- Conduct patient assessments, monitor vital signs, evaluate breathing, manage pain, and administer medications and treatments.
- Ensure medical equipment and supplies are accessible.
- Educate families about prognosis, care plan changes, medications, and hospice goals.
- Provide compassionate, patient-centered care in a home setting.
- Maintain compliance with licensing and safety standards.
Requirements
- Valid, active RN license in Michigan.
- Minimum of one year of relevant RN experience, with community health and Medical-Surgical background preferred.
- Reliable transportation, valid driver’s license, and current insurance required.
