Hospice Registered Nurse Case Manager (RN)

About the role
The Registered Nurse (RN) will provide care and services for a group of patients, visiting them where they reside—including private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, offering expertise in psych-social assessments, interventions, and knowledge of community resources.
You will work closely with patients on a one-to-one level while receiving strong support from a patient care team. The role requires independent work with daily activities such as phone, email, and written communication, meetings with healthcare professionals, and an understanding of hospice rules and regulations.
Responsibilities
- Provide direct nursing care to hospice patients in various settings (private homes, Skilled Nursing Facilities, assisted living facilities).
- Collaborate daily with nurses, social workers, chaplains, volunteers, and aide leadership in a team environment.
- Conduct psych-social assessments and interventions, and share knowledge of community resources.
- Manage time effectively to balance visits, documentation, and meetings, prioritizing and triaging multiple concerns.
- Use an electronic medical records system for documentation and communication.
- Travel across the service area with reliable transportation, a valid driver’s license, and auto insurance.
Requirements
- Current Registered Nurse (RN) licensure in good standing in the state of Ohio.
- Ability to work independently while managing day-to-day activities, including phone, email, and written communication.
- Strong time management skills to schedule visits, attend required meetings, and respond to urgent needs.
- Willingness to drive and travel across the service area with reliable transportation, a valid driver’s license, and auto insurance.
