RN Case Manager
Responsibilities
- Complete an initial assessment of patients and their families to determine hospice care needs.
- Regularly re-evaluate patients' nursing needs.
- Use assessment data to develop and update individualized care plans.
- Collaborate with the interdisciplinary healthcare team, patients, and families to ensure coordinated, continuous care.
- Counsel patients and families regarding end-of-life care and support.
- Initiate appropriate preventive and palliative nursing interventions.
- Communicate with physicians regarding patient needs and report changes in condition.
- Supervise Hospice Aides.

Starting Salary: $93,600

Qualifications
- Associate Degree in Nursing required
- Current State RN license
- Minimum of two years of professional nursing experience; hospice experience preferred
- Valid driver's license and reliable transportation
- Willingness to travel
- Excellent verbal and written communication skills
