Hospice Registered Nurse Case Manager (RN)

About the role

The Registered Nurse (RN) at Suncrest Hospice will provide care and services for a group of patients, visiting them in their homes, Skilled Nursing Facilities, and assisted living facilities. This role is a contributing member of the interdisciplinary team, focusing on psychosocial assessments, interventions, and knowledge of community resources.

Responsibilities

• Provide care and services to patients in various settings including private homes, Skilled Nursing Facilities, and assisted living facilities.
• Collaborate with the interdisciplinary team to ensure comprehensive patient care.
• Work independently and on a 1-1 level with patients, supported by a patient care team.
• Manage time effectively to prioritize visits, documentation, and meetings.
• Learn and utilize an electronic medical records system.
• Drive with reliable transportation, a valid driver's license, and auto insurance to cover the service area.

Requirements

• Current Registered Nurse (RN) licensure in the state of Ohio.
• Works well within a team environment, collaborating daily with nurses, social workers, chaplains, volunteers, and aide leadership.
• Willingness to drive with reliable transportation, a valid driver's license, and auto insurance.

Qualifications

• Strong communication and interpersonal skills.
• Experience in hospice care or related field preferred.

Skills

• Excellent time management skills.
• Knowledge of hospice rules and regulations.
